Fisher & Paykel Forma Full Face Mask Seal & Foam Kit

This kit replaces the worn cushion and seal on your Fisher & Paykel Forma full face mask, so you get a fresh, leak-resistant fit without buying a whole new mask. It includes one FlexiFoam foam cushion in the size you select (Small, Medium, Large, or Extra Large) and one silicone seal, and it fits the Forma frame in any size, since all four cushion sizes work with the same frame.

The Forma cushion uses a two-part design: the inner FlexiFoam layer contours to a wide range of face shapes and helps keep the mask aligned on the frame, while the outer silicone seal stays soft and light and actively contours to your face for a secure seal. If you only need to replace the foam, we also carry the Forma foam cushion on its own, and if your straps are worn too, pair this kit with new Forma headgear.

We recommend replacing the cushion and seal about every 1 to 3 months: if the foam looks flat or squished on your face, that's your sign it's due for a swap (and possibly the headgear too).
